This notice went into effect on January 05, 2026. We will limit the uses or disclosures to the minimum necessary. We reserve the right to change the terms of this notice and to make the new notice provisions effective for all PHI that we maintain. A copy is available on request.
I. Your Rights as a Client
You have the right to ask questions about any methods used during Therapy, Christian counseling, or Life Coaching. You have the right to decide at any time not to receive Therapy, Christian counseling, or Life Coaching from New Insight 2 Life. We may be able to provide you with the names of other qualified professionals whose services you might prefer upon your request. You have the right to end Therapy, Christian counseling, or Life coaching at any time without any moral, legal or financial obligations other than those already accrued.
II. Confidentiality
Within certain limits, information revealed by you during Therapy, Christian counseling, or Life Coaching will be kept strictly confidential and will not be revealed to any other person or agency without your permission. At times, Therapy, Christian counseling, or Life Coaching will involve the participation of more than one family member and/or significant persons. If we are doing Couples or Group counseling, while we will attempt to follow your wishes, we do not guarantee confidentially among participants in the therapy, Christian counseling, or Life Coaching process.
There are certain situations in which we, as therapists, Christian counselors or Life Coaching professionals, are required by law to reveal information obtained during Therapy, Christian counseling, or Life Coaching to other persons or agencies without your permission. These situations include:
If you threaten bodily harm or death to another person, we are required by law to inform the intended victim and appropriate law enforcement agencies.
If you threaten bodily harm or death to yourself, we will inform the appropriate law enforcement agencies and others (such as spouse, friend, or an inpatient psychiatric institution) who could aid in prohibiting you from carrying out your threats.
If you reveal information related to current abuse or neglect of a child, dependent adult, or elderly person, we are required by law to report this to the appropriate authorities.
Social Networking and Internet: We do not accept friend requests from current or former clients on our psychotherapy related profiles since these sites can compromise clients' confidentiality and privacy. For the same reason, we request that clients do not communicate with us via any of these interactive websites.
III. Christian Counseling Services and Fees
An individual therapy, Christian counseling or Life Coaching session is between 50 - 60 minutes. A family, couple, or group’s Christian counseling session is typically 90 minutes. We typically see clients on a weekly or bi-weekly basis; however, we may see each other more than once a week or every other week depending on need. If you are unable to attend your scheduled appointment, you must call at least 48 hours in advance or you will be charged the full fee for the missed session.
Payment is required at the time of your appointment, unless other arrangements have been made in advance. If at any point during therapy, Christian counseling or Life Coaching you are unable to pay your fee, please communicate this to staff and fee arrangements may be renegotiated.
For adult individuals: our fee is $100 per 60-minute session.
For children age 17 or younger: our fee is $80 per 50-minute session.
Couples therapy or Christian counseling is $150 per 60-minute session.
Couples therapy or Christian counseling is $200 per 90-minute session.
Family therapy or Christian counseling is $250 per 90-minute session. For Groups, the fee is $50 per group session.Sliding Fees are available based on financial need upon discussion and agreement with therapist, Christian Counselor/Life Coach. Please note, initial intake session are usually 60 minutes. we receive payments in electronically via checking account, debit, credit card or flex spending accounts. Please make checks payable to Sharon M. Jones, PhD., or New Insight 2 Life Coaching, Counseling, LLC. (Clients assume a $35.00 penalty fee associated with NSF checks. If there is a fee increase, we will give you at least one-month notice beforehand.)We an often be reached at 321-290-0343 Monday through Friday 5pm to 9pm. We usually return calls within 24 hours. If you have a counseling emergency or are feeling suicidal after hours, please call the 24-hour Emergency Crisis line in your county or 911.
Email and phone communication are typically reserved for scheduling communications only. As the confidentiality of email is uncertain, We do not conduct therapy, Christian counseling or Life Coaching by email. We typically respond to email within 48 hours during the workweek, unless we are unavailable for any reason. If you are canceling an appointment with less than 48 hours notice, please leave a voicemail message, text or email. IV. HIPPAA Notice
Uses for therapy, Christian counseling or Life Coaching, Payment, and Health Care Operations:
We may use your Protected Health Information (PHI), for treatment, payment, and health care operations purposes with your consent. The “consent” is given when you sign the Christian counseling/Life Coach - Client Services Agreement.
Client's Rights and Counselor's/Life Coach Duties: Client’s Rights:
Right to Request Restrictions – You have the right to request restrictions on certain uses and disclosures of PHI. However, we are not required to agree to a restriction you request.
Right to Receive Confidential Communications by Alternative Means and at Alternative Locations – You have the right to request and receive confidential communications of PHI by alternative means and at alternative locations. (For example, you may not want a family member to know that you are seeing us. On your request, we will send your statements and communications to another address.)
Right to Inspect and Copy – You have the right to inspect or obtain a copy (or both) of PHI in our therapy, Christian counseling or Life Coaching and billing records used to make decisions about you for as long as the PHI is maintained in the record. We may deny your access to PHI under certain circumstances, but in some cases, you may have this decision reviewed. You may inspect and copy therapy, Christian counseling Notes unless we make a therapy, Christian counseling or Life Coaching determination that access would be detrimental to your circumstances. On your request, we will discuss with you the details of the request and denial process.
Right to Amend – You have the right to request an amendment of PHI for as long as the PHI is maintained in the record.
We may deny your request. On your request, we will discuss with you the details of the amendment process.
Right to a Paper Copy – You have the right to obtain a paper copy of the notice from us upon request, even if you have agreed to receive the notice electronically. The cost is 75 cents per page.
Therapy, Christian counseling or Life Coaching Duties:
We are required by law to maintain the privacy of protected health information regarding you and to provide you with notice of our legal duties and privacy practices with respect to PHI.
We reserve the right to change the privacy policies and practices described in this notice. Unless we notify you of such changes, however, we are required to abide by the terms currently in effect.
If you are concerned that we have violated your privacy rights, or you disagree with a decision we made about access to your records, you may discuss this with us at this office.
V. Observation and Release of Information
By signing below, I give my consent. I understand that every attempt will be made to hold my confidentiality. This includes changing names and identifying information when discussing cases during supervision. I understand that any supervisor, Christian counselor, therapist, life coach, or therapist-in-training who discusses my case is under the same confidentiality requirements as my therapist, counselor/ life coach. Furthermore, I understand that if by chance any supervisor, christian counseling, therapist, life coach, or therapist-in-training knows me socially, he/she will immediately leave the session and will not observe, seek, or be given any information about my case. I also understand that the purpose of allowing discussion of my christian counseling or life coaching sessions is to enhance the effectiveness of the therapy, christian counseling or life coaching I am receiving with New Insight 2 Life. I understand that I may withdraw this consent at any time.
This release is valid for one year from the date of signature and submission of this form.
